Police responded to the collision on Old South Head Road at 1.10am and said several cars had been extensively damaged.
A witness said one car was ripped in half. He said that one car was embedded in the side of another.
Police allege the man, from Kensington, returned a positive roadside breath test.
He was arrested and taken to Waverley police station, where police say he was found to be four times over the legal blood alcohol limit.
He was charged with high-range drink-driving and will face Waverley Local Court on May 6.
The man and his 35-year-old male passenger were treated at the scene for minor injuries.
